PROBLEM TO BE SOLVED: To provide an information medium which indicates specified prices and makes the recorded information surely readable at a high speed. SOLUTION: The information medium consists of nearly a circular disk or a polygonal disk having at least two pairs of sides parallel to each other and having the lengths of the two sides of the respective pairs equal to each other. A hologram 1 which is mainly composed of plural concentric circles by a reference beam made incident perpendicularly on the one surface of the disk and is reconstitutable to patterns 3 around the reference beam 2 as a center axis is formed in the central part or at the intersected points of two intersecting symmetrical lines on at least the one surface of the disk.
