The main body of the implant is joined to a core (2) and guide sleeve (4) which have corresponding positioning elements (22,23;24,25). The guide sleeve has a central opening (6) for containing at least part of the core, and or the core is fixed, preferably at least approximately without play, radially in the guide sleeve. The exterior of the sleeve has a radial constriction (10). The axial length (14) of the guide sleeve is at least 50, preferably 70, % of the core's length (12).
