FIELD: medicine.SUBSTANCE: vein is dissected to the required length, removed from the wound, stretched to the utmost along the entire length for atraumatic passage of valvulotome. DeBakey needle is placed into the proximal lumen of the vein dissected for shunt, the proximal end of the vein is temporarily fixed to oliva by ligation on the needle. Syringe containing a mixture of saline solution and heparin at 36-37 °C is connected to the end of the needle. Then folded valvulotome LeMaitre is inserted into the lumen through the distal end of the autovein. Autovein valves are destroyed with the valvulotome. Valvulotome is lead to the autovein proximal valve, opened and by injection of warm saline solution with heparin make the required straightening of the autovein lumen from valve to valve and resection of the valves by valvulotome reverse traction on completely straight-line autovein bloated with warm solution. Quality of valvulotomy is evaluated using thermovisor as the valvulotome is pushed forward.EFFECT: method enables valvulotomy in a small diameter vein with minimum damage of autovein wall and control of crossing of all valves in autovein.2 cl, 1 ex |
