The instrument consists of a guide (1) which is positioned in contact with the femur on a level with the trochlea, and a rotary cutter which operates when pressed against the guide to remove a section of the trochlea. The guide has a recess (3) formed by a rim (2), the inner wall (4) of which guides the rotary cutter as it is pressed against the outer edge (5) of the rim. The rim (2) has an inner edge (6) which matches the shape of the end of the bone, and the surface of revolution bounded by the inner edge (6) of the guide corresponds to the shape of the femoral implant. At least one portion of the inner edge (6) can have a cutting edge, and the guide has one or more lugs (8a, 8b, 8c) to receive fixing pins.
