PROBLEM TO BE SOLVED: To provide a receive carrier for form disassembly capable of disassembling a form installed on a bottom of a beam or a slab without damaging a floor surface by a sleeper, the form, a pipe and the like, scattering the pipe, and generating noise when the form is disassembled. SOLUTION: This receiver carrier 1 for form disassembly is used when the form 2 installed on the bottom of the beam or slab S is disassembled and consists of an uppermost part 3 provided in such a way that a length of an inclined part is expanded and shrunk freely in the oblique direction and provided with a projecting member 3a in its lowermost end part, upper support columns 4a, 4a, 4b, 4b supporting the uppermost part 3, lower support columns 5, 5, 5, 5 fitted in the upper support columns 4a, 4a, 4b, 4b so as to solid freely in the vertical direction, a box part 6 which is protrusively provided with the lower support columns 5, 5, 5, 5 in upper corner parts, respectively, and whose upper face is opened to receive and hold constituent members of the form 2 which drop, and leg wheels 7, 7, 7, 7 supporting the box part 6 movably.
