A surgical device (10) is provided for forming a channel through or into tissue utilizing both mechanical energy, and laser energy. The device includes a needle (40) for forming part of the channel. The needle has an open bore (46). Mounted within the bore is an optical fiber (77) for emitting laser energy to form the balance of the channel.
