PROBLEM TO BE SOLVED: To provide a proof forming apparatus with which the color tone of an object printed material can be accurately and stably reproduced in a relatively simple structure and dots are easily checked, or to provide a method for forming a proof. SOLUTION: The apparatus forms an area gradation image as a proof of a printed material in the form of an ensemble of dots on the basis of the digital image data of the multicolor printed material. The apparatus is equipped with: a color separation table which stores two or more kinds of dot color data with similar color tones to each other assigned to correspond to at least one print color; and a control output part which outputs the signal to form the dots on the basis of the color separation table and the digital image data.
