a plurality of electrodes to deliver a pacing therapy; and a processor configured to determine a pacing threshold in response to the delivered pacing therapy, determine whether there is a change in the pacing threshold, and determine patient prescription drug intake compliance in response to the determined changes, wherein the processor is configured to generate an alert in response to the determined patient prescription drug intake compliance, deliver pacing therapy subsequent to generating the alert, determine a pacing threshold in response to the pacing therapy delivered subsequent to generating the alert, determine whether there is a change in the pacing threshold determined in response to the pacing therapy delivered subsequent to generating the alert, and determine patient medication compliance of prescription drug intake in response to determining whether there is a change in the pacing threshold determined in response to the pacing therapy delivered subsequent to generating the alert.
