The device has a shock wave therapy source, a coupling arrangement for introducing the generated shock waves into the patient and a positioning device. The positioning arrangement has a light source (8) and corresp. lens (6) arranged in a guide tube (7). The guide tube is inserted into the therapy source (5) so that an image of the illuminated place (2) on the patient which is to be treated is formed on a display arrangement (9,10) or directly observed through the lens. The light source has an associated fibre glass bundle light conductor arranged on the shock wave axis for both transferring illuminating light and the focussed image.
