PROBLEM TO BE SOLVED: To eliminate need of checking an address printed on a mail against those printed on certification sheets of mail delivery such as a certificate of delivery and its copy by separably placing the certification sheets on a part of a rear sheet of an envelope. SOLUTION: The envelope with a certificate of delivery 1A has address information of a receiver printed on a mail form as well as printed in respective address columns 10a, 11a of a certificate of delivery 10 and its copy 11 indicated on a rear of an envelope lower sheet 5 provided on a part of the mail form, wherein the information is printed approximately simultaneously. In addition, since bar codes 10c, 11c based on a code number assigned to a large user or the like by the Ministry of Posts and Telecommunications are printed on respective strips, the envelope, the certificate of delivery 10 and its copy 11 do not disagree in the printed information. Therefore, it is not necessary to check the address on the certificate 10 and the copy 11 separately prepared from the envelope.
