Even when the beam for servo is different from the beam for recording/reproducing information, the relative positional relationship between the focus and tracking can be maintained constant. The optical storage medium includes a reflection surface for reflecting a first beam having a first wavelength and a second beam having a second wavelength and an information storage layer where the first beam comes and the information is recorded as an interference pattern or the wave front according to the interference patter is reproduced as the information. The reflection surface has a plurality of marks or guide grooves formed thereof so as to be used for tracking operation or focus operation when the second beam is applied. The information storage layer has a reference interference pattern partially formed in advance for use as a reference for correcting the relative positional relationship between the first beam and the second beam when the first beam is applied to the information storage layer.
