PROBLEM TO BE SOLVED: To provide an image forming apparatus capable of acquiring a satisfactory image with initial data by extracting an identifier from a read image and linking the identifier with electronic data. SOLUTION: The image forming apparatus comprises an image reading unit 10 for emitting lights from a first direction and a second direction to acquire an original and image data of an image formed on a recording medium by the original; a conveying path for conveying the recording medium having a fixed toner image formed thereon from the second direction to an irradiation position to be irradiated with the light; an identifier extracting unit 21 for extracting the identifier of the original and the identifier of the recording medium having the image formed thereon from the image data; and a storing unit 18 for storing the identifiers so that they are correlated with the corresponding input data.
