<P>PROBLEM TO BE SOLVED: To improve the recording density of a hologram recording medium of an optical waveguide type. <P>SOLUTION: A lens L3 formed to, for example, a hemispherical shape is provided in a position proximate to the surface of a hologram memory 11 to condense signal light B1 in a core layer 2. At this time, a gap which is below the wavelength of the signal light BS is provided between the surface of the hologram memory 11 and the lens L3, and thereby the signal light BS is prevented from being totally reflected even when the signal light BS is made incident by a wider incident angle than before, and the recording of the signal is made by making the signal light BS reach the interior of the hologram memory 11.
