Use of prostacyclin or an analogue thereof for treatment of acute critically ill patients with raised levels of thrombomodulin or adrenaline or syndecan-1, in particular acute critically ill patients with systemic endothelial damage. Thrombomodulin, adrenaline or syndecan-1 above a threshold levels as biomarkers for identifying critically ill individuals that have a medical indication for treatment with prostacyclin.
