PROBLEM TO BE SOLVED: To provide an image processing program, device and method, capable of easily controlling the smoothing strength or emphasizing strength without determining complicated parameter in the adaptive filtering process, and obtaining a stable and high-quality medical image by reducing the image noise while inhibiting generation of mosaic pattern noise. SOLUTION: The image processing device comprises a local area setting part 11b for setting a local area for each focused pixel of the medical image, a parameter determining part 11d for determining the width and number of the pixel value intervals for generating a histogram indicating the frequency of appearance of the pixel values in the local area, a histogram generating part 11e for generating the histogram, an operation interval setting part 11f for setting an operation interval in the histogram, and a filtering part 11g for weighting the frequency of appearance of the pixel value interval to the median of the pixel value interval as the relative weight for each pixel value interval included in the operation interval.
