PROBLEM TO BE SOLVED: To provide a one-part type delivery slip with seal, which is designed to have a seal for designating handling of a load to be adhered to an area other than the surface of the delivery slip for a delivery load.SOLUTION: The delivery slip with seal is a one-part type delivery slip, in which a printing substrate 11 is laminated by pseudo adhesive or strong adhesive with a tack substrate 21 of a tack sheet 20 composed of a tack substrate 21, an adhesive layer 22 and a release sheet 23, except seals S1 and S2 for designating handling of a load printed on the surface of the tack substrate. The seals S1 and S2 for designating handling of a load, with the periphery punched through by a half-cut line h4, can be removed from the tack substrate 21. In addition, the surfaces of the seals S1 and S2 for designating handling of a load are weakly adhered to the printing substrate 11, and the periphery, along the half-cut line, of the seals for designating handling of a load is not adhered to the printing substrate. |
