A method for evaluating renal status in a subject, comprising: obtaining a urine sample from a subject; performing an assay method configured to detect Trefoil factor 3 by introducing the urine sample obtained from the subject into an assay instrument that (i) contacts all or a portion of the urine sample with a binding reagent which specifically binds for detection Trefoil factor 3, and (ii) generates an assay result indicative of binding of Trefoil factor 3 to the binding reagent; correlating the assay result to the renal status of the subject by using the assay result to assign the patient to a predetermined subpopulation of individuals having a known predisposition of a future acute renal injury, wherein the known predisposition of a future acute renal injury is a probability that the future acute renal injury is more or less likely to occur within 24 hours of the time the urine sample is obtained, the assignment made by comparing the assay result or a value derived therefrom to a threshold selected in a population study, wherein the threshold separates the population into a first subpopulation above the threshold which is at an increased predisposition for recovery from acute renal failure relative to a second subpopulation below the threshold; and treating the subject based on the predetermined subpopulation of individuals to which the patient is assigned, wherein if the patient is in the first subpopulation, the treatment comprises one or more of renal replacement therapy, withdrawal of compounds that are known to be damaging to the kidney, delay or avoidance of procedures that are known to be damaging to the kidney, and modification of diuretic administration.
