PROBLEM TO BE SOLVED: To prevent hooking of the tonguelike film part of a three-dimensional film shaped clamp plate with the frame of a frame-shaped clamp plate. SOLUTION: In a preform mold having the clamp plate 4 pressing the film 2 introduced into the preform mold capable of obtaining a three-dimensional film by squeezing the film 2 into a preform recessed surface 3 to the predetermined place of the periphery of the preform recessed surface 3, the clamp plate 4 is opened and closed like a door. The clamp plate 4 consists of a lower frame- shaped member and an upper member for preliminarily holding the film in an annular shape before pressing the film 2 introduced into the preform mold to the predetermined place of the periphery of a preform projected surface.
