<P>PROBLEM TO BE SOLVED: To provide a display medium, an information collection system and an image forming apparatus which allow users who do not have professional knowledge to accurately notify the maintenance department of a manufacturing company of the state of the image forming apparatus. <P>SOLUTION: The information collection system is composed of a communication apparatus which has a function to read two-dimensional bar codes and communication functions, the image forming apparatus which has network connection functions and communication functions, and an information processing apparatus which has network connection functions and communication functions. The communication apparatus encodes first information for device state collection of the image forming apparatus and reads the two-dimensional bar code displayed on the display medium and transmits the first information included in the read two-dimensional bar code to the image forming apparatus. The image forming apparatus collects device state information corresponding to the first information received from the communication apparatus and transmits it to the information processing apparatus. <P>COPYRIGHT: (C)2008,JPO&INPIT |
