PROBLEM TO BE SOLVED: To manufacture a combination form of a transfer blank and a return postcard and secure the confidentiality of information to be entered in the return postcard. SOLUTION: This combination form is constituted of a postcard piece P3 and a transfer sheet piece P2 of a prescribed transfer blank size, separably connected to a form 11-1 equivalent to a twelvemo weighing 70 kg in terms of ream, and the postcard piece P3 is formed of a postcard main body section p1 and a postcard concealment section p2. In addition, a release paper 34 is attached to the surface of an adhesive layer 33 applied to one region P31 which is located on a valley side of a fold 16 of the postcard main body part p1 and a concealment label 36 with a re-release paste layer 36b is attached to the entire surface of the postcard concealment part p2. Further, the folded halves of the postcard main body part p1 are laminated with the adhesive layer 33 in such a way that both halves cannot be separated and thus the prescriptions of the postcard are met. Information can be concealed by releasing the concealment label 36 of the postcard concealment part p2 and releasably attaching the concealment label 36 to an information entry column 22 provided on the face of the postcard main body part p1.
