PROBLEM TO BE SOLVED: To provide an identification medium enabling multi-stage identification including visual inspection.

SOLUTION: An inventive identification medium comprises, in order from its reading side: a cholesteric liquid crystal layer 102 which is subjected to hologram processing 103 and reflects circular polarization light in a first turning direction selectively; and a code display layer 107 on which a code to be read is formed, is normally invisible, and emits fluorescence by irradiation with ultraviolet rays to be visualized. A hologram image inhibiting reading of information of the code when the hologram image and the code are simultaneously read is formed by the hologram processing.
