The treatment instrument is provided with: a sheath having a longitudinal axis and provided with a lumen for insertion of a guide wire along the longitudinal axis, a first opening that is in communication with the lumen and provided at the distal end thereof, and a second opening that is in communication with the lumen, provided at the proximal end thereof and formed in the outer circumferential surface; a proximal end holding section holding a proximal region of a proximal portion of the sheath; and a distal end holding section that bends and holds a distal region of the proximal end portion of the sheath, and is contiguous with the distal end of the proximal end holding section and inclined with respect to the proximal end holding section at an angle such that the second opening is located on an extension of the central axis of the lumen extending in the distal region. |
