PROBLEM TO BE SOLVED: To form a document made of many recording sheets which can certainly prove originality. SOLUTION: Different specific images are respectively formed on a plurality of recording sheets to the recording sheets to be outputted in a predetermined order. A confirmation hole is formed to meet the position of the specific image to the recording sheet at the upper position from the recording sheet in which the specific image is formed. When the document in which these recording sheets are stacked can be seen from above, the originality of the document can be proved when the specific image is seen through the confirmation hole. If the specific image cannot be seen from the confirmation hole, the change of the document can be understood by the dropping, replacing, etc. of the partial recording sheet, and the originality of the document cannot be proved.
