PROBLEM TO BE SOLVED: To provide a form with a sticking slip which can be cut-open easily without impairing a label, and a manufacturing method thereof. SOLUTION: Form sheets for sticking to articles sticking slips whereon prescribed items are printed beforehand are joined continuously in the direction of flow of continuous paper with perforation lines 14 provided between and a label 15 with an adhesive is stuck releasably to the form at least on either side, on each perforation line. In a form 10 with the sticking slip thus constituted, the perforation line 14 is formed of a line of consecutive perforations each shaped like a letter 'Y'. By shaping the perforation like the letter 'Y', it is unnecessary to make the ratio of cut portions large as in a usual form, and the form with the sticking slip can be manufactured by an ordinary process.
