An apparatus (1) for positioning a medical instrument (2) with respect to a target tissue (3) comprises reference means (9) with a plurality of guide channels (11) with inlet openings (12) in a distal surface (10) to allow the medical instrument (2) to pass in a guided manner, and visual signaling means (29) which generate a visual signaling on the distal surface (10) in dependency of a spatial relationship between the reference means (9) and the target tissue (3) to assist in the positioning of the medical instrument (2) with respect to the target tissue (3) through a selected opening (31) of the inlet openings (12).
