PROBLEM TO BE SOLVED: To provide an information input and output system suitable for carrying, without causing image quality in formed images to deteriorate, and without having articles of consumption for output on paper durable in folding, lightweight, and superior in portability. SOLUTION: A sheet-like medium 14 prepares a discrimination code image that is an image obtained by coding discrimination information capable of specifying a document, a code pattern image 11, including a coordinate code image that is an image coding position information indicating a position on the sheet-like medium, a document image 12 formed on the code pattern image 11, and an output region image 13 indicating the output region capable of adding electronic information.
