A slab form apparatus is provided to facilitate the deconstruction of the slab form apparatus by enabling the outer head move down on the circumference of the inner head easily without rubbing. A slab form apparatus comprises a slab form(100) with a bent part(110) on the periphery, a middle beam(200) with a pin(210) or plate, and a column(300) including an inner head which is tapered downward and an outer head(344) formed on the outer side of the inner head. The outer head moves down smoothly without rubbing with the inner head, and is equipped with a first seating member(348) with an inclined surface is placed on one side and a second protrusion(360) with an inclined surface(362) on the other side.
