PROBLEM TO BE SOLVED: To provide an optical filter module which is compact and easily aligned. SOLUTION: The optical filter module is provided with a filter substrate which is composed of a plurality of substrates which are laminated and have reflection faces which reflect light of corresponding wavelengths, respectively, in a way that the reflection faces are aligned in parallel to one another. One end parts of a first optical fiber and a plurality of second optical fibers are optically connected to the filter substrate on the same side in terms of the respective reflection faces, the first optical fiber is arranged so that the axial direction has an angleαwith respect to the normal direction of the respective reflection faces and the plurality of second optical fibers are arranged on the same plane as that of the first optical fiber so that the axial direction has an angleαwith respect to the normal direction of the respective reflection faces on the opposite side of the first optical fiber with respect to the normal direction of the respective reflection faces.
