Disclosed is a mold form apparatus which is able to improve work convenience and secure work efficiency. The mold form apparatus comprises: a rail unit which has a first rail and a second rail which are arranged in a state of forming an arrangement of two rows by being erected to correspond to the construction direction of a tower structure; a variable frame unit which is installed to be able to move along the first and second rails of the rail unit and has a first support frame and a second support frame which form a horizontal frame unit capable of adjusting a size while sliding in a state of forming a plane mutually during a movement; and a holding plate unit which is arranged on the horizontal frame unit formed by the first and second support frames of the variable frame unit.
