A drive transmission device comprising: a driving shaft; a shaft supporting member configured to support the driving shaft rotatably; a member to be attached to which the shaft supporting member is attached; and a rotary member attached to the driving shaft and configured to transmit driving force, wherein the member to be attached includes a positioning hole configured to position the shaft supporting member, wherein the shaft supporting member includes: a first bearing portion with which a first bearing surface configured to support the driving shaft is provided; a second bearing portion with which a second bearing surface configured to support the driving shaft is provided; a connecting portion configured to connect the first bearing portion and the second bearing portion; a positioning portion engaged with the positioning hole for positioning the shaft supporting member with respect to the member to be attached; wherein the first bearing portion, the second bearing portion, the connecting portion, and the positioning portion are integrally formed, wherein a position of the first bearing portion and a position of the second bearing portion are different from each other in axial direction of the driving shaft, and wherein a center of axis of the first bearing surface, a center of axis of the second bearing surface, and a center of the positioning portion are disposed on substantially the same straight line.
