The present invention relates to an image processing device, image processing method, and surgical system whereby a region including a specific living body part in an intraoperative image can be detected on the basis of the frequency of a movement vector of the living body in the intraoperative image. In the present invention, a movement detection unit detects a movement vector of a living body in an intraoperative image using an intraoperative image from a different time. An analysis unit calculates the frequency of the movement vector detected by the movement detection unit. A frequency map generating unit detects a region including a specific living body part in the intraoperative image on the basis of the frequency calculated by the analysis unit.
