The workflow is traversed based on a changing of the context of the patient to determine appropriate actions. Relevant data to send for coordinating activities in real-time for providing care to the patient is determined based on the appropriate actions. A relevant user of the plurality of users is determined based on the contexts of the plurality of users and the appropriate actions. The relevant data is sent to the relevant user.
