<p><P>PROBLEM TO BE SOLVED: To eliminate an illegal claim in a care insurance system, to reduce office work man-hours of a care service provision organization related to a care insurance claim, and to make examination of an examination organization rapid and proper. <P>SOLUTION: Falsification of a care execution situation of a care execution date or execution contents is disabled and the care execution situation is recorded in an electronic recording medium 12 by a recording device 11, and the electronic recording medium 12 is recovered to the care service provision organization 3. The electronic recording medium is read by a reader 3A and is transmitted to a care insurance execution management organization 4, or the recording medium is sent as it is. The care insurance execution management organization prepares a model of a care premium payment bill, and prepares a payment claim for a care premium wherein the care service provision organization prevents the falsification, by use of the model.
