PROBLEM TO BE SOLVED: To provide an authentication device, an authentication system, an authentication method and a program for controlling the authentication device for permitting a reliable visitor to perform access to a network with confidential information while securing sufficient security without imposing any labor on a system manager. SOLUTION: An authentication device outputs an application form 60 to be used for the permission application of access on which an identification image 606 showing information including the identification information of terminal equipment is printed. Then, the authentication device reads the application form 60 whose writing has been performed by a user as necessary, and recognizes the information from the image data acquired by reading, and permits the terminal equipment corresponding to the recognized information to perform access to a network.
