PURPOSE:To make the production efficient by continuously feeding out the preform whose head part is preformed, partially heating the head part of the preform while under the horizontal transportation, then forming it by picking up on the center of the header forming part. CONSTITUTION:The preform 1 whose head part is preformed is fed to a horizontal transportation device 20 by feeding one by one from a feeding device 10. The preform 1 is held by a pair of the right and left holding parts 22 provided on a conveyer 21 and the head only is heated by passing the heating device 40 equipping a heater 42 as well. It is then picked up by a transfer chuck 50, transferred to a header forming part 30 and formed. With the device of this composition the production can be performed efficiently.
