PROBLEM TO BE SOLVED: To surely discharge smoke out of a peritoneal cavity by providing an aeroperitoneum means having a gas injecting means for injecting the gas into the peritoneal cavity to swell the peritoneal cavity and a sucking means for sucking the gas in the peritoneal cavity, and driving the aeroperitoneum means when a smoke generated by the cauterizing treatment of an affected tissue by a smoke detecting means. SOLUTION: In the medical treatment of the inner part of a peritoneal cavity under endoscopic observation, an aeroperitoneum device 8 is set, and CO2 gas is injected into the peritoneal cavity through a trocal 9 to expand the peritoneal cavity. A valve unit 27 is controlled by a control part 28 to regulate the temperature, flow rate or the like of the gas supplied from a bomb 26. When the internal pressure of the peritoneal cavity reaches a prescribed value, an endoscope 2 or a high frequency treatment tool 12 is inserted into the peritoneal cavity through trocals 9, 10, a high frequency cauterizing device 7 is started to cauterize the affected tissue in contact with the cauterizing electrode 29 of the high frequency treatment tool 12.
