PROBLEM TO BE SOLVED: To provide a face image information forming device, a face image photographing card manufacturing method, a card manufacturing device, a card manufacturing method and an information forming method for quickly, simply, inexpensively and accurately carrying out the issuance of a card with a face photograph and the like. SOLUTION: A photographing means for photographing a person to be photographed and obtaining image information, an image information storage means for storing the image information and a proper information input means for inputting the information proper to the person to be photographed are provided and also an image information forming means for forming the image information with the proper information of the person to be photographed to be attached for reference of the information proper to the person to be photographed and the card manufacturing means for manufacturing a card with a face image based on the output from the image information forming means are provided.
