PURPOSE: To perform demolding without damaging a lens or a form member by controlling the lever action acting an the upper half of a lens form by the lift pattern extending to the peripheral part of the lens form to separate the upper half of the form from the lower half thereof in an inclined drawing-up direction. CONSTITUTION: A lens taking-out and form separating apparatus 10 is arranged behind the baking oven 22 for polymerizing a lens arranged to the form assembly on a pallet 20 freely movable along a transport route and provided with the operation mechanism of a steam heating device for heating a lens form and also provided with a mechanism mutually separating the fitting of an operation mode and the lens form to take out an integrally molded lens. After a contact lens is polymerized, the quantity of heat of steam or laser energy is added to the rear curved form 40 of the form assembly 50 and the rear curved form 40 is peeled from a front curved form 30 by one set of bending fingers to take out a lens.
