PROBLEM TO BE SOLVED: To obtain an inhalation preparation containing a specific cephem compound having an antibacterial action or its salt as an inhalation agent and capable of expressing an extremely high therapeutic effect by the administration of a small amount of the inhalation preparation. SOLUTION: This inhalation preparation contains a cephem compound of the formula [R<1> is a (protected)amino; Z is N, CH; R<2> is a (protected)carboxy (lower) alkyl; R<3> ,R<4> are each a lower alkyl; A is a lower alkylene; R<5> is a (protected)hydroxy] or its medicinally acceptable salt as an inhalation agent. The form of the inhalation preparation includes an aerosol preparation, a powdery inhalation preparation, and a liquid inhalation preparation. For example, the aerosol preparation is prepared by finely grinding the cephem compound or its salt preferably into a size of <=5μm, if necessary, adding a dispersant to the ground product, and subsequently charging the mixture into a spray container together with a propellant under cooling. The inhalation preparation is effective e.g. for intractable or relapsing respiratory tract infections caused by Pseudomonas aeruginosa, etc.
