PROBLEM TO BE SOLVED: To provide a form for producing a concrete molding which can exactly reproduce an original design and is reduced in weight and a method for producing the form and to provide a form for producing the concrete molding which can be used repeatedly and a method for producing the form. SOLUTION: A form design forming part has a shape for forming the design surface of the concrete molding having a surface design and is composed of open-cell hard polyurethane foam.
