MEDICATION LIST GENERATOR

Methods, systems, and computer-readable media are provided for collecting information regarding the medication an individual is taking before an individual's healthcare visit. A patient medication list is generated using scanned barcode information or information from a photograph taken with the patient's mobile device. The pre-populated medication list is served to a clinician during the individual's healthcare visit without the need for manual entry of each medication the patient is currently taking to speed up the medication reconciliation process.

1. A system useful in a computer healthcare system serving a prepopulated medication list for an individual, the system comprising:
(a) a computer store containing data, for each of a plurality of medications, defining a plurality of visual perceptible elements, which visually perceptible elements correspond to medication and identification of an individual;
(i) wherein each of the plurality of medications includes medication information;(ii) wherein the identification of an individual includes an identification code that has been linked to an individual's computing device; (b) a computer server at the healthcare information system, which computer server is coupled to the computer store and programmed to:
(i) receive from the user device a first signal indicating a first barcode that has been scanned;(ii) automatically decoding the first source medication from the barcode that has been scanned;(iii) automatically identify the identification code of the individual from the first signal;(iv) in response to identification of the first source medication, automatically populating a medication list with the first source medication information; and(v) receive from the user device a second signal indicating a second barcode that has been scanned;(vi) automatically identify the second source medication of the barcode that has been scanned;(vii) automatically identify the identification code of the individual from the second signal;(viii) in response to identification of the first and second source medication, automatically populating a medication list with the second source medication; and(ix) using the populated medication list and the identification codes from the first and second signals to generate and serve to a browser window that displays: the visually perceptive elements for the first and second source medication of the bar code scan with the identification code of the individual.
