A new cephem compound of the formula: wherein R<1> is amino or a substituted amino group; R<2> is carboxy or a protected carboxy group; R is a group of the formula: -A1-R<3a> [in which A1 is hydroxy(lower)alkylene, amino(lower)alkylene, protected amino(lower)alkylene, lower alkenylene or hydroxyimino(lower)alkylene wherein the hyrogen atom of the hydroxyimino group may be replaced with a lower aliphatic hydrocarbon group; and R<3a> is carboxy or a protected carboxy group] or a group of the formula: [in which A2 is lower alkylene which may have an oxo group; and R<3b> is organic group which may have suitable substituent(s)]; and Q is hydrogen or lower alkoxy; and pharmaceutically acceptable salts thereof and processes for their preparation and also a pharmaceutically composition comprising, as an effective ingredient, the above compound in association with pharmaceutically acceptable carriers.
