The instrument has a domed end portion (3) which is insertable in the cotylar cavity and includes an internal piston (6) sliding within an aperture of the dome. A flange (7) extends around the base of the dome, and includes a number of peripheral projecting pins (11) to engage in holes in a flange surface of the cotylar cup. The flange receives a rotatable plate (16) in its central portion, which is removably attachable to a manual gripping tube (23). The tube incorporates a control shaft (24) sliding axially within the tube, actuated by a handle (25) at the end of the tube, for detaching the dome from the flange portion and rotatable plate.
