The loop for the thumb is linked to a coupling for the actuator wire by a mechanism that translates thumb movement into coupling movement, allowing fine instrument adjustments to be carried out. The device (4) comprises a cylindrical tube (8) with a thumb loop (10) attached to it, this loop is capable of being moved in the axial direction. A coupling (14), also movable in an axial direction, is attached to the tube and linked at one end to a wire (604) used to activate the instrument mechanism. A mechanism (36) linking the thumb loop with the coupling converts axial movement of the loop into axial movement of the coupling.
