PROBLEM TO BE SOLVED: To provide an integrated label type index card, facilitated in inserting into a cartridge. SOLUTION: The integrated label type index card 24 is received into an opening and closing type cartridge receiving case, formed of a parallelopiped constituted of a main body having a bottom wall as well as a lid having an upper wall and provided with a cartridge inserting opening at the front surface of the same. The index card is provided with a back surface unit 40 by bonding one side of the index card, which is treated through separation treatment, and one side of the label, which is treated through adhesive treatment, to abut the same against the back wall of the cartridge receiving case, a main surface unit 33, continued to the back surface unit through a bent part 37, and a turning surface unit 35, continued to the main surface unit through the bent part. Both ends of the bent part provided between the main surface unit and the turning surface unit are cut obliquely to make slanted rims 52. Only the area of the label surface side of an area 36 near both sides of the bent part provided between the main surface unit and the turning surface unit are removed.
