Disclosed is a system for tracking medical data. The system is comprised of a computing environment having a processor and a database in data communication with the processor. The computing environment is configured to aggregate medical data from disparate sources, structure the aggregated data into the database, invite one or more members of a care team to view and provide medical data about a particular patient, identify a source of each item of the provided data, and provide for display on an electronic display a plurality of data items corresponding to the particular patient in a single screen with unique indicia for each data source. The displayed data may be in the form of a timeline showing the medical data along its length.
