A medical resection instrument uses ultrasonic vibrations for resectioning tissue from a living organism while the body cavity concerned is observed by an optical observation pipe (8). The resection instrument (2) has an ultrasonic vibrator (14) for generating ultrasound waves, a conical horn (18) connected to the vibrator (14) and a probe (12) connected to the front horn section for transmission of the ultrasonic vibrations. The probe is formed with a rod-shaped shaft (12a) and a distal terminal section with a resection knife (13), an electrical supply unit (11) supplies a control voltage to the vibrator (14) and the optical observation pipe (8) has a guide hole (6) in which the shaft (12a) of the probe (12) is inserted. <IMAGE>
