An incision instrument of the scalpel type comprises a handle (1) with a blade attached through the head (10) and a sheath (3) capable of being moved between two positions, one where it sits over the handle (1) and the other where it covers the blade. According to the invention, the instrument incorporates retaining means to lock the sheath (3) in each of the said positions, consisting specifically of an open groove (11) on the handle (1) and projections (30, 31) on the sheath (3) co-operating with the said groove (11). Application particularly to surgical instruments of the scalpel type.
