System and method for Mobile Platform Designed for Digital Health Management and Support for Remote Patient Monitoring

A mobile application is used for monitoring and management of users or patients with various health or disease conditions. Software system provides a platform with which the medical histories, the recent conditions and real-time measurement data for the patient can be organized and shared among various people who are involved in the caring of the patient. In addition to data sharing in a secured, private networking environment, the platform integrates the essential functions for people in the various caregivers' groups to communicate with each other in real-time so as to collaborate on the caring of the patient.

1. A personal mobile health management system, comprising:
a mobile application running on a mobile device with wireless connectivity; personal health records for at least one individual stored on a mobile device running said mobile application; a database system on said mobile device to store personal health records; an alert management module to alert at least one caregiver group based on unique alert levels; a permission management module to manage who can see each personal health record; a software module integration between said mobile application and sensors to receive sensor data; and a data processing module to process received sensor data, to update the personal health record and to evaluate health updates to determine if an alert is needed.
