a substrate comprising an upper substrate and a lower substrate; and an electrode provided on the lower substrate, wherein the upper substrate, the lower substrate, and the electrode are formed of an optically transparent member, and wherein the substrate displays identification information of the sensor by a pattern formed by using at least one of a transmitting portion provided on the surface of the substrate that allows light entering the substrate to pass through and a light-blocking portion provided on the surface of the substrate that blocks light entering the substrate.
